Bipolar disorder is often treated with lithium carbonate. In fact this has been the standard treatment for the disorder for more than fifty years. Lithium helps to reduce mood swings, and in particular has a noticeable effect on the more extreme symptoms. Lithium works by minimizing the ‘manic’ part of the bipolar cycle, either preventing it or lessening the intensity of the symptoms.
Lithium has certain advantages as a treatment. For instance, lithium is fast acting, typically only taking a week to have an effect. For bipolar disorder patients who have seen their lives suffer badly before they are correctly diagnosed, this can be one of the greatest benefits of lithium – no need to wait another three months before they can get their lives back on track.
Since it is the oldest drug used in this treatment, and generic versions of the drug are available, lithium is relatively cheap. Lithium is available under a number of different brand names, including Lithobid and Eskalith. Newer drugs are still paying for the cost of their development, and are still under patent, giving the drugs firms a monopoly and the ability to set a higher price. At the same time, newer drugs are being promoted heavily by drugs companies and their representatives. This has a cost, and potentially makes them seem more attractive to medical professionals than they really are.
Bipolar disorder is not a condition that can be ‘cured’; it is a long term condition. Because of this, it is important that patients on a course of lithium do not stop taking the medication because they feel well; this can lead to the resumption of mania. It needs to be taken on a regular basis, normally daily.
Lithium is safe for long term use, with a long history of successful treatment of bipolar disorder. Some individuals have been on lithium for over twenty years – it was first used in the US in 1970. However, lithium can affect the kidneys, so tests of kidney function should be carried out bi-annually to ensure damage is not done. (For the same reason, patients on lithium should ensure they drink plenty of water.)
In fact while some of the drugs used in treating bipolar disorder are very new, lithium has been use for hundreds of years; naturally occurring lithium salts were given to patients to lick, in an attempt to ameliorate their condition. Fortunately drug delivery systems have become more professional since those days! Lithium is now also available in extended-release tablets, which phase the delivery of the chemical over a period of time. This can improve a patient’s reaction to the treatment.
Most patients taking lithium do not need to adjust their lifestyles or worry about side effects. However, women who are pregnant or are considering having a baby should discuss this with their doctors, as lithium medication carries various risks to both mother and child during pregnancy and breastfeeding.
Other drugs used in the treatment of bipolar disorder include Trileptal and Lamictal.
Lamictal is still under patent and is manufactured by Glaxo SmithKline. It can provide mood stability for as long as 18 months, and reduces the likelihood that bipolar persons will need to take additional medication to treat individual mood episodes (eg anti-psychotic drugs). Lamictal is said to be particularly effective at delaying the onset of depression.
Trileptal is also known as oxcarbazepine, and is manufactured by Novartis. It is used for a particular subset of bipolar disorder patients who have very severe symptoms and whose manic episodes involves micro seizures similar to those seen in epilepsy.
Whatever the drugs and other therapies chosen, it is important that people affected by bipolar disorder seek treatment. While there is no ‘cure’, the symptoms of bipolar disorder can be greatly alleviated by lithium and other medications, and patients can look forward to a normal and happy life.
